Dear Sir,

This refers to my Savings Account No 47208 with your Bank. This is to bring your kind notice that I had issued a cheque No 875329 dated the 22nd of May, 2016, for an amount of 10,000/- in favour of Mr Mamun. He informed me that he has not received the cheque till now as it has been lost. I sincerely request you to cancel the cheque and please stop the payment, if and when it is presented to your bank. Please confirm me that you have taken necessary action in this regard.
